Financial Responsibility, Appointment, Cancellation & Late Arrival Policy

Thank you for choosing Bloomfield Health and Wellness for your healthcare needs. We are committed to providing exceptional, personalized care while respecting the time of every patient and maintaining an efficient schedule. To ensure the highest quality experience for all patients, we ask that you review and acknowledge the following office policies.

Payment Policy

Payment is required prior to the rendering of services unless otherwise arranged in advance.

Bloomfield Health and Wellness is a direct-pay practice for consultations and most in-office services. We do not bill or accept medical insurance for provider visits, consultations, or most saliva-based laboratory testing.

If laboratory blood work is ordered, you may have your specimens collected at the laboratory of your choice. In many cases, blood testing may be covered by your medical insurance; however, coverage varies by plan. It is the patient's responsibility to verify eligibility, benefits, deductibles, copayments, and coverage directly with their insurance carrier prior to testing.

For your convenience, we accept:

  • Credit and debit cards
  • Cash
  • Personal checks (where applicable)
  • Other approved electronic payment methods

Payment must be received before services are provided.

Cancellation & No-Show Policy

Because appointment times are reserved exclusively for you, we respectfully request at least 24 hours' notice if you need to cancel or reschedule your appointment.

Appointments cancelled with less than 24 hours' notice, or appointments in which the patient fails to arrive without prior notification ("no-show"), prevent other patients from receiving timely care.

A $100 no-show/late cancellation fee will be charged to the card on file for:

  • Failure to attend a scheduled appointment.
  • Cancellation with less than 24 hours' notice.
  • Repeated late arrivals that result in the inability to complete the scheduled appointment.

Patients with repeated no-shows, excessive late cancellations, or chronic tardiness may be required to prepay for future appointments or may be dismissed from the practice at the provider's discretion.

Late Arrival Policy

We understand that unexpected delays occasionally occur. If you anticipate arriving late, we ask that you notify our office as soon as possible.

Patients who arrive after their scheduled appointment time may receive a shortened visit in order to remain on schedule and avoid delaying other patients' appointments. Any portion of the appointment that cannot be completed due to a late arrival may need to be rescheduled for another date.

To protect the quality of care provided to all patients, the provider reserves the right, at their sole discretion, to cancel or reschedule an appointment if a patient arrives late and the remaining appointment time is insufficient to safely or appropriately complete the scheduled service without adversely affecting subsequent appointments.

If an appointment must be rescheduled due to a patient's late arrival, the visit may be treated as a late cancellation and applicable cancellation or no-show fees may apply, at the provider's discretion.

Repeated late arrivals may result in limitations on future scheduling privileges or dismissal from the practice.

Financial Responsibility

Patients are responsible for all fees associated with services rendered by Bloomfield Health and Wellness.

By scheduling and attending an appointment, patients acknowledge responsibility for payment of all applicable charges, including consultation fees, laboratory fees not covered by insurance, cancellation fees, and no-show fees, as outlined in this policy.
